Default keep losing alternator belts

My alternator keeps coming off. (obviously I have a v-belt setup).
I tried to move the alternator out a little more by inserting washers behind the bracket bolts.
The alternator pulley now seems lined up with the crank pulley but I'm still losing alternator belts for some reason.
Has any else had this problem or know how I can fix this problem?
