Does anyone know it a rear disk conversion from a 88 SVO mustang will directly adapt to a 87 GT? I know the axles need changed to 5 bolt, but what about everytning else (calipers, back plates, E-brake, soft lines).

uhh, Paul, I hope that is a typo. There is no such thing as an '88 Mustang SVO. They were last made in '86.
You have a number of options when converting to rear discs. First, and probably the easiest, is to buy a complete kit from Motorsport/FRPP (whatever they are called now), Stainless Steel brakes, etc. Everything needed in the kit is included for around $500. Second, buy parts from a '94 or newer. This will require 5-lug axles and wheels, but, again, is a direct swap. Third, buy parts from a Thunderbird turbo coupe, Lincoln, etc. This may be a bit more difficult, but will work equally as well as the others. The 8.8 rear end is used in many cars besides the Mustang, so parts inter-changing shouldn't be a problem.

You can do what I did, and get the parts from an 87-88 turbo coupe, you can switch over the hardware and axles or just bolt on the whole rear end.
