Question T5 problem

I have a problem with my T5. It works fine except when I shift out of 4th there is what sounds like gear clatter. It goes into 4th just fine but it makes a noise comming out of it.That is the only gear that does it. I have checked my fluid level and it is fine. Is there any adjustment that needs to be made or have I got big problems. The T5 is out of a 1990 Mustang and has about 55,000 miles on it. When I first installed it it worked perfectly. I only have about 45 miles on the car now. Can anyone give me a hand? Thanks for your help in advance.

P.S. I was the guy who earlier had a balance problem and many of you helped me figure that one out. Thanks again.
