You are correct... its not SUPPOSED to happen.... but I've seen it happen twice. Both sidewinders too.... Seen a nosewheel tire go the same way too.... Aircraft stopped... One tire didn't....
As to why it happens, I'm no Aviation Ordnanceman (AO) but I would think it would be a malfunction within the pylon or the ordnanceman who mounted it made a mistake.
Trust me. There's a lot of explaining to do when your AIM-9 decides to take a drink!
I've also seen a gas cap fly off of an external tank after catching a wire spraying JP-5 (fuel) right in front of the starboard intake.
Fuel goes into intake... and you've got an AIRCRAFT FIRE!
Ahhh yes, nothing like being paid an E-4's salary while making sure the sidewinders, sparrows, and phoenix missiles don't cook off....
