Remember when it was just music videos? I do. I remember the first night it came out. It was such a big deal, and it was cool. At the time, I lived in a small part of the San Fernando Valley that was in dispute between 2 or 3 cable companies as far as who got to "serve" that area. The result was no cable at all for several years after everyone else had cable. A friend of mine's sister was house-sitting for a couple in Encino, and we all went over there to watch the unveiling of MTV. It was a really big deal.

As far as the Real World, I have been known to watch it sometimes when there's nothing else on, if I kinda trip over it while channel surfing. For me, it pretty much tells me how out of tune I am with "kids" today. If I woke up tomorrow and was 18 again, I think I'd be screwed. At least in the MTV world.
