Default Help with codes please

Ok I think I have fixed all of my codes but these two, 12 & 41 both KOER. I have already changed the O2 sensor but I am still getting the code what else could it be? As for the idle one what do I clean?
How do I make all these dam codes go away. I just want that sweet 11 to pop up and nothing more before I install the new heads and intake.
