Default Rocker bolts

I have been working on my friends 1995 mustang and the we are attempting to tighten the rocker arm bolts. The book only says - Step 1: 45 ft lbs - Step 2: 19. That's all it says. I understand the 45lbs, but what is the 19 for. It does not say what to do with the 19. Do I need to loosen the bolt and retighten to 19 lbs or is it trying to say to tighten to 45 and turn an add. 19 degrees. It just is not specific, any help appreciated.
