Default Trick Flow TW head valve seals coming loose

Any idea what would cause this? The heads have 3000 miles on the rebuild with bronze guides and they are not worn out at all. One of the springs that clamps the seal to the guides broke and wound up in my oil pump somehow and siezed it up, destroyed my dizzy gear and chipped a tooth on my cam. They are the white teflon seals. Could they have been improperly installed?
