Default Torque Converter gone splat?

has anyone ever heard of a Torque Converter decided it doesn't want to engage or disengage anymore?

I have a 90' Mustang GT with 180,000+ miles and orginal TCC ... does anyone think it would be possible for this to malfunction on its own? or would I have to abuse the car first.
