Looking for help. I have a '96 mustang and have had the rotors turned three times and replace once in the past 40,000 miles. I can't figure out why they are warping so badly. Any suggestions?

first tell whoever re-mounts your tires to NEVER EVER EVER let me re iterate NEVER EVER EVER use a impact gun to do so, over tightening the lug nuts will warp a rotor in quick order...they should be spun on by hand, tightened initially by hand and then torque wrenched into place to factory specs...

did i mention to NEVER EVER EVER put them on with an impact gun?

Normaly rotors warp because of I heat (i.e. excessive hard braking). When you replace your rotors did you also replace the brake pads? If not you need to have your rotors re-surfaced and get new brake pads. Also be easier on the brakes ;-)
