There is an operating range for the tensioner. Take a look at the front face of it, there is an indicator that should be pointed within a range that is marked on the tensioner. If it is not, I would replace it.

I suppose it is also possible that the pulley bearing could wear out causing some movement right at the pulley. Just take a close look at it and see if you can find excessive wear.
