The clear coat is not properly bonded with the color coat. This could be caused by a number of reasons. From severe oxidation, to poor prep work, to incompatability between the coats, as mentioned above. It can also be caused by using non-clearcoat safe products on your paint, or spilling of liquids like brake cleaner, fluid, or gasoline onto the surface.

There is no easy fix for the situation that I'm aware of. It will simply get worse and worse until the clear coat is removed, and the car is repainted.

If you had it painted by a shop, I suggest contacting them to have them re-do it.
