just a guess, but it seems to me that it could have something to do with the one o2 sensor looking bad and the other looking good. obviously it appears that one side is not flowing the same as the other. i wuold have replaced them both as you cant really tell just by looking if its good or not. (clean doesnt always mean good)
you might try changing spark plugs, and other basic tuneup items.
i wouldnt think that one o2 would be clean and the other filthy though, that doesnt sound right.
