The cam being worn out could make a clicking noise but it wouldn't be the piston hitting the valve. It that is occuring you will also need new valves because the one being hit will be bad. If the cam went flat, the lifter on the lobe that went flat will make alot of noise. Could be alot of other things also but I would say it's not piston to valve noise. Unless there was a big lift cam put in there recently. Good luck.
