Does this problem happen all the time, or only when you are in gear?

If you have a torque converter code and this only happens when you are in gear, I'd be seriously suspect of the converter itself. Like it may be trying to lock up or something. I've seen that happen and kill a car just like not letting out the clutch and coming to a stop.
