Question Engine blueprint form?

I was talking to my machinist today about blueprinting my engine. He told me that although he would be happy to notate all the measurements he didn't have any sort of form to fill out. After we talked for a while he told me that if I could find him a good form and make a few copies for him, he'd do the blueprinting for free.

Now I'm stuck because after 15 minutes of "google" searches I can't seem to find a form. Does anyone have one? If nothing else, let me see your blueprint so I can make my own. I'd hate to let an opportunity like this pass me by.
