Sounds like you have improperly adjusted shifter stops, something wrong with the clutch system, or bad shift forks inside the transmission.

Do you have an aftermarket shifter with stops on it? If so, that would be the first place I would check. The next would be trying to get it into those gears while stationary. When it was popping out of gear it sounds like you weren't getting it far enough into gear to hold.
