Default centerforce clutch problems

when i first installed the clutch it was fine, it was only after about 6 months i noticed it started to get hard to get it in any gear at a dead stop. Then if i got it in gear and rev'd it up over 3k it would try to start to pull the car forward. I have an adjustable cable on the firewall side which i adjusted to no avail. Any suggestions.
