I would first check with local windshield replacement places, they should be able to order a windshield. Carlite (search the web) is the official Ford glass supplier but I don't know if they retail to the public.
Fixing the dent on the windshield frame could be tricky, can you take the car to a body shop? If the frame is not bent, you may be able to use a slide hammer with a great deal of care. Welding studs to the dented area and pulling on them with the slide hammer is preferred to the old practice of drilling holes, inserting screws to pull on.
