I'll take a wild guess and say, rough shifts, occational grinding while shifting.

The T45 doesn't have the greatest syncros in the world, especially the 96-98s. They tend to wear very quickly even under resonable driving. But attempting to force the transmission into 1st at 45-50 mph, rather or not he managed to actually get it into gear, would not help the situation at all either.
